At Peptide Doc Medical, patient safety and responsible medical care are our highest priorities. All prescription medications provided through our telehealth platform are subject to medical evaluation, clinical appropriateness, and provider discretion.

By using our services, you acknowledge and agree to the following Prescription Policy.

Medical Evaluation Requirement

Prescription medications are prescribed only after completion of required intake forms, review of medical history, health screening, and evaluation by a licensed healthcare provider to determine medical appropriateness and treatment eligibility. Treatment recommendations are based on professional clinical judgment, patient safety considerations, contraindications, potential medication interactions, applicable medical standards, and overall medical appropriateness. Submission of payment or medical intake information does not guarantee prescription approval, medication eligibility, dispensing, or continuation of

treatment.

Provider Discretion

All prescribing and treatment decisions are made solely at the discretion of the evaluating licensed healthcare provider. Providers reserve the right to approve, deny, modify, discontinue, or delay treatment based on clinical findings, patient safety considerations, laboratory results, medical history, potential contraindications, or applicable regulatory requirements. Patients may also be required to complete additional evaluations, obtain laboratory testing, provide updated medical records, or participate in follow-up consultations before prescriptions are issued, renewed, or continued.

Prescription Fulfillment

If treatment is approved, prescriptions may be transmitted to affiliated licensed pharmacies, including Optio Rx and Boom Rx. These pharmacies operate independently and are solely responsible for compounding, dispensing, packaging, shipping, and compliance with applicable pharmacy laws and regulatory obligations. Peptide Doc Medical does not directly manufacture, compound, or dispense medications.

Compounded Medications Disclosure

Some prescribed medications may be compounded

medications prepared by licensed pharmacies. Compounded medications are not FDA-approved, may differ from commercially available medications, and are prescribed only when deemed medically appropriate by the evaluating healthcare provider based on clinical judgment and patient-specific needs. Patients are encouraged to discuss all potential risks, benefits, side effects, and available treatment alternatives with their healthcare provider prior to starting treatment.

Controlled Substances

Peptide Doc Medical may limit, restrict, or decline the prescribing of certain medications, including controlled substances, based on applicable telehealth regulations, patient safety considerations, state laws, clinical

appropriateness, and provider discretion. Not all

medications or treatments are available through telemedicine services.

Prescription Renewals and Refills

Prescription renewals and refills require ongoing provider oversight and may depend on treatment response, adherence to the prescribed plan, side effect monitoring, follow-up compliance, updated health information, laboratory review, and continued medical appropriateness. Providers reserve

the right to modify, suspend, delay, or discontinue prescriptions at any time if continued treatment is determined to be medically inappropriate, ineffective, or unsafe.

Patient Responsibilities

Patients agree to provide accurate and complete medical information, use medications only as prescribed, avoid sharing medications with others, promptly report any adverse reactions or concerning symptoms, and comply with all provider recommendations, monitoring requirements, and follow-up care instructions. Failure to comply with treatment recommendations or safety requirements may result in suspension, discontinuation of care, or loss of prescription eligibility.

No Guarantee of Outcomes

Peptide Doc Medical does not guarantee weight loss results, symptom improvement, cosmetic outcomes, or specific treatment success. Individual responses and treatment outcomes vary based on numerous factors, including medical history, lifestyle habits, nutrition, exercise,

adherence to treatment recommendations, underlying healt

conditions, and other individual health-related factors.

Prescription Shipping and Processing

Prescription processing and shipping times may vary

depending on provider review, pharmacy processing timelines, medication availability, applicable state regulations, and shipping carrier conditions. Once a prescription has been transmitted to an affiliated pharmacy for fulfillment and processing, orders generally cannot be canceled, modified, or refunded.
