  • A prescription will be renewed;

  • Any particular treatment will be recommended;

  • A healthcare provider will determine that treatment is medically appropriate.

Medical decisions remain solely within the professional judgment of the treating healthcare provider.

WE DO NOT GUARANTEE THAT:

  • THE SERVICES WILL BE UNINTERRUPTED;

  • THE SERVICES WILL BE ERROR-FREE;

  • ANY MEDICAL OUTCOME WILL BE ACHIEVED.

16. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, PEPTIDE DOC MEDICAL, ITS AFFILIATES, OFFICERS, EMPLOYEES, CONTRACTORS, PROVIDERS, AND AGENTS SHALL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES.

OUR TOTAL LIABILITY ARISING FROM OR RELATED TO THE SERVICES SHALL NOT EXCEED THE AMOUNT PAID BY YOU TO PEPTIDE DOC MEDICAL DURING THE TWELVE (12) MONTHS PRECEDING THE CLAIM.

SOME JURISDICTIONS MAY NOT ALLOW CERTAIN LIMITATIONS, SO PORTIONS OF THIS SECTION MAY NOT APPLY.

17. Indentification

You agree to defend, indemnify, and hold harmless Peptide Doc Medical and its affiliates, officers, employees, contractors, providers, and agents from claims, damages, liabilities, losses, costs, and expenses arising from:
